Physical and History Forms

A new physical must be completed each school year. Physicals for the 2025-2026 school year MUST be dated ON or AFTER May 15, 2025

Physicals may be completed by your Primary Care Physician, a drop in clinic, PA (Physician Assistant), NP (Nurse Practitioner), and DO (Doctor of Osteopathic Medicine). Physical pages must be stamped by the doctors office.

Home Campus

To start your online clearance, please visit homecampus.com.

Incoming freshmen may use their middle school ID numbers and change them to their Woodcreek student ID number later. Allow no less than 72 hours for processing or longer at the end of the school year. Student athletes will be cleared by season of sport.An athlete must be cleared through Home Campus prior to summer activities, conditioning, practice, tryouts or games.

Clearance Prerequisite

  • Students must be clear of all fees/fines/book returns prior to participating in sports and being cleared to participate in sports. 
  • Physical & History Form must be updated each school year for your physical appointment. All four forms must be uploaded and it is easiest to upload two at a time. 
  • Have your student athlete's insurance card (not the parent's card) ready to upload. If you don't have medical insurance, you may purchase it by following this link.

Capital Valley Conference (CVC) Game Schedules
CIF-SJS - Transfer Student Athletes - Transferring from one school to another school may affect a student's athletic eligibility under SJS of CIF rules.  It is your responsibility to see the Athletic Director or school designee for a copy of the eligibility rules, or information where eligibility rules may be viewed.  Students who transfer as a result of disciplinary action are subject to the conditions of Bylaw 209 and may be ineligible for one calendar year of enrollment in your new school.  All eligibility transfer information/ applications MUST be true, correct, accurate, complete and/or not false or fraudulent.  Go to www.cifsjs.org for further information about transfer eligiblity.
