You can download a Work Permit Request Form above or pick one up at the Career Center. They are the yellow forms that are hanging on the wall by the door.

Reminder: you need to receive a job offer before you request a work permit.

Instructions

  1. Submit your completed Work Permit Request Form to Mrs. Lott in the Career Center by 3:00pm for next day processing.
    • The following sections must be completed:
      • Student info with SSN
      • Employer info and signature
      • Parent signature
    • All signatures must be wet ink signatures. Electronic signatures cannot be accepted, per state of CA law.
    • An incomplete Work Permit Request Form may delay the processing time of your permit.
  2. Pick up your official work permit from the Career Center the next business day at lunch or after school. Students must be present to pick up.
To qualify for a work permit, you must currently have a 2.0+ GPA and/or no more than 1 F grade, and satisfactory attendance. If you do not qualify, admin and/or counselor approval will be needed before Mrs. Lott issues the work permit.
